Big Sky Private Case Management (Big Sky PCM) creates outstanding quality of life improvements for the aged, ageing and their carers. We are committed to ensuring access to client-centred care, quality of life and dignity as the highest priority for our select group of private clients.

Big Sky PCM provide services to a select group of senior private clients with complex community care needs (and their primary carers) by planning and delivering bespoke care management plans and associated services. These include:

- Comprehensive Initial Assessment and Care Plan Development;
- Comprehensive Cognitive Assessment and Specific Dementia Screening;
- Activities of Daily Living Review and Functional Independence Screening;
- Occupational Therapy Home Assessment; and
- Client Support Services.

Currently we provides first-class, bespoke services to clients in Canberra, located in the Australian Capital Territory.

Places are limited and, in most cases, you will need a referral from a General Practitioner or Medical Specialist.

It takes time, experience, expertise and dedication to deliver high quality service on an individual basis – limiting the number of people that can be assisted at any one time.
