Acmns Aged Care Medical Nursing Specialists

We are a mobile qualified team of Nurse Practitioners, allied health and geriatrician's providing support and services to the residents at the Residential Aged Care Facilities. We provide education and clinical competencies to the Registered Nurses and case conferences to the residents and families. We provide bulk billed service and consultations for older person in their home or at the Residential Aged care Facility. The Nurse Practitioner works collaboratively with the General Practitioner in ongoing care of the residents.
