ccCHiP was formed in 2008 as a collaboration between Concord Centre for Mental Health, Concord Hospital Department of Endocrinology and Metabolism, and Sydney University. We hope to improve metabolic health and prevent cardiometabolic complications by targeting modifiable risk factors in high-risk patients. We aim to help clinicians across disciplines gain the skills to meet the preventative health needs of their patients. We are also working on a range of clinical and educational tools in this area. Since 2008, our service has operated an integrated, multidisciplinary clinical service at three sites across SSWAHS, screening and detecting metabolic disorders among consumers with mental health diagnoses, and initiating preliminary treatment. We are now commencing development and implementation of coordinated management programs.
