DREAM (Distributed Research in Emergency and Acute Medicine) is a revolutionary research and evidence based approach dedicated to improving the practice of emergency care. We are emergency specialists and experienced researchers collaborating with ambulances, emergency departments and related institutions across Australia. The DREAM Collaboration is a new approach to a problem, which is still only partially recognized. We know that Emergency Departments (ED) and Hospital information systems are good at delivering administrative and demographic data, but are poor at providing clinical detail. We link these datasets using deterministic linkage techniques to give a continuous patient record from ED triage, through admission as an inpatient, to discharge from hospital. In order to complete this continuous record and to enable accurate epidemiology, predictive modelling and the estimation of disease prevalence, our researchers add a small amount of granular clinical detail from the emergency department to each record. We plan to form a true collaboration not only of institutions and health organisations, but also of individual clinical researchers. We plan to use data effectively to enable and promote outcomes-based emergency medicine and resuscitation practice thus empowering working clinicians to ask and answer questions they see as vital.
