💥QUADS & ACL INJURY💥 ——— 🔎In a group of 15 professional soccer players who had been cleared to play following ACL reconstruction (on ave 8mths post-op), only 3 passed the accepted cutoff of 90% limb symmetry on 3 seperate quad strength tests (Herrington et al, 2018). Interestingly, two players passed the accepted 90% cutoff for single leg hop & triple crossover hop respectively. - 📌This has some big implications for ACL rehab. Grindem et al (2016) showed that for every 1% dec...rease in quad strength below 90%, knee injury risk increased by 3%. So, if you're returning to sport with 80% limb symmetry, your risk of reinjury is 30%. - ✅Secondly, "functional exercises" can hide quad strength deficits that persist following ACLR. Thus, the reason why we need to isolate the quads via knee extensions to compliment the "functional" compound rehab exercises. - 🏋🏼♂️Lastly, if professional athletes (who get daily medical, physio and strength & conditioning) can't achieve the best practice guidelines of 90% on a battery of strength and function tests by 8-9 months following ACLR, non-professionals shouldn't expect a return to sport around that timeframe. - ➡️Our suggestion to all non-professionals is wait at least 12 months AND pass the discharge criteria before returning to sport 💪🏻

✅You just had a BAD ankle sprain. It's black and blue. How do you know that its ONLY an ankle sprain, and not anything worse like a FRACTURE?! Well, lucky for you, there's a quick an easy test you can do NOW to screen if you need to go to the emergency department and get an x-ray. . ANSWER THESE QUESTIONS: . 1️⃣ Can you take 4 steps (it's okay if you need to limp)? NO? --> GET AN X-RAY... . 2️⃣ Do you have tenderness/pain around your medial or lateral malleoli (the little bone bumps on either side of you ankle)? YES? --> GET AN X-RAY . 3️⃣ Do you have tenderness/pain around the base of the 5th metatarsal (bump on the lateral/outside portion of your foot; halfway between your heel and your little toe)? YES? --> GET AN X-RAY . 4️⃣ Do you have tenderness/pain around your navicular bone? (bump on the medial/inside portion of the foot, underneath your ankle crease)? YES? --> GET AN X-RAY . 🔎This battery of screening tests, called the Ottawa Ankle Rules, has a sensitivity (ability of a test to rule out a diagnosis) of 100%, which means if you CAN take 4 steps AND you DO NOT have tenderness in the specific areas mentioned here, it almost certainly means you DO NOT HAVE AN ANKLE/FOOT FRACTURE and, thus, do not need an x-ray!! . 📌Now, if you CANNOT take 4 steps or DO have tenderness in those areas, this DOES NOT NECESSARILY MEAN YOU HAVE A FRACTURE - it just means you should get an x-ray. The specificity (ability of a test to rule in a diagnosis) of the Ottawa ankle rules is low. It is a SCREEN designed for RULING OUT, not to RULING IN a fracture.
Your physiotherapist or GP are able to refer you on for an x-ray or as mentioned above go to your emergency department if you believe you have an acute ankle/foot fracture. •••••••••• 📚Bachmann LM, et al. Meniscal Tears
The meniscus of the knee (lateral and medial) is a thickened crescent-shaped cartilage pad between the two joints formed by the femur (the thigh bone) and the tibia (the shin bone). The meniscus acts as a smooth surface for the joint to move on whilst providing increased joint stability.
A meniscus tear can be a source of knee pain, whether this be a direct result of trauma, the ageing process or a combination of the two. Many other knee structures may present... similarly to a meniscus injury, so correct diagnosis is vital.
How does a physio determine if the meniscus is causing your knee pain? Via a thorough patient history, which includes checking for locking and giving way during daily activities, a history of twisting while weight bearing and hyperflexion of the knee. Examination may include looking for joint line tenderness, minimal to moderate synovial swelling, pain on forced flexion and limited extension with spring block end feel. We may refer for an MRI in cases of a high level of suspicion.⠀⠀ ⠀⠀ Treatment? Surgery? Conservative Management?
Surgery may be appropriate in certain cases, particularly those experiencing instability or locking of the knee. Age, level of activity and chronicity of symptoms will also determine which treatment pathway is appropriate. There has been a large push for conservative management (i.e. physio) in those with degenerative meniscus tears with exercise therapy proving to be as effective as surgery.
Physiotherapy aims to improve pain free range of motion, increase muscle strength and proprioception and return patients to their previous level of function. This is delivered generally by a combination of education around the injury and load management, manual therapy e.g. massage and joint mobilisation and most importantly exercise prescription!
