Interchange Australia provides care and support services people to live independently.

Our services are designed to meet individual needs to enhance independence, health and wellbeing.

Disability Services
National Disability Insurance Scheme (NDIS)
Interchange Australia is an approved provider of supports under the National Disability Insurance Scheme across NSW. The NDIS provides eligible people under 65 years of age with funding to purchase support services, therapies and equipment from approved providers.

Aged Care Services
Community Home Support Programme (CHSP)
Interchange Australia is an approved provider of Commonwealth Home Support Programme (CHSP) services in the Wingecarribee Shire. CHSP services are available for people over 65 years of age and their carers who are mostly, but not completely, able to live and cope on their own. Interchange Australia is funded to provide personal care, domestic assistance, individual and group social support and respite.

Home Care Packages Programme
Interchange Australia is an approved provider of Aged Care Services including Home Care packages. These packages provide older people who want to stay at home with access to a combination of supports that assist with day-to-day activities. Support services include personal care, domestic assistance, gardening, home maintenance and modifications, social support and transport. The packages also include access to nursing, allied health and other clinical services, care coordination and case management.

Hospital and Home
Interchange Australia is an approved provider of Hospital and Home programmes including ComPacks and Healthy at Home. The ComPacks programme provides up to six weeks support to assist people discharged from a public hospital to recuperate at home and prevent readmission. The Healthy at Home programme also provides up to six weeks support to eligible people to prevent a hospital admission.

Interchange Australia is also an approved provider of support services for people accessing a Transitional Aged Care Package (TACP). This enables people, following a hospital stay, to receive a combination of support services and nursing and allied health services for up to 12 weeks to facilitate physical and psychosocial rehabilitation goals.

Navigation and Service Assistance
Interchange Australia can provide consultation to individuals and /or their carers to navigate the aged and disability care systems and source individually tailored solutions to current and future care needs. Our consultants have many years’ experience and up-to-date knowledge of aged care and disability services to ensure people get the best support available whether through government-funded or private services or a combination of both.

Additional services
We provide a range of private services including but not limited to assistance with showering and grooming, medication monitoring, cleaning, shopping, transport to appointments, accompanying people to social activities, respite, meal preparation, overnight support and hospital to home support. We can support anybody at any life stage with any health condition and /or disability for any length of time.

Our staff
We have a team of qualified staff who are experienced in providing care and support services. We match staff abilities and personalities to clients’ requirements to ensure we meet the required needs. All staff have Working with Children Checks, National Police clearances, first aid certificates and receive regular training and supervision. We have an on call team available to manage services after hours and on weekends and public holidays.

Our quality management
Interchange Australia has a quality framework for managing and delivering services. We are governed by organisational policies and procedures as well as the NSW Disability Services Standards pertaining to people under 65 and the Home Care Standards pertaining to people over 65. We provide clients and carers with a statement of rights and responsibilities and information on lodging complaints.

Requesting services
NDIS
If you are a registered NDIS participant and wish to purchase services from us please contact our head office. If you want to investigate becoming an NDIS participant visit www. ndis. gov. au or call 1800 800 110.

CHSP and Home Care Packages Programme
To receive CHSP services or a Home Care Package you will first need to register with My Aged Care on 1800 200 422 or visit www. myagedcare. gov. au so you can be screened for eligibility. If eligible you will be referred for a home assessment and your assessor can refer you to Interchange Australia for services.
